Person-centered thinking is an ongoing process of listening and learning; focused on what is important to someone now, and in the future. It recognizes that people will grow and change over time and so the support and services they agree to now will not stay the same forever.
You can use the following person-centered tools to help you celebrate what’s going well and work out what needs to change:
What is working/not working
You can use the “working/not working” tool on a day-to-day basis to identify what needs to be done to improve things. It can help you to analyze a situation from multiple perspectives.
Learning logs
You can use the learning logs tool after a specific event or activity to find out what went well, what didn’t work well and what you have learned for the future.
4 plus 1 questions
You can use the 4 plus 1 questions tool to take a braod look at multiple aspects of a person’s life, project or issue and gather a team’s collective learning to determine the next steps.
