Steven’s perfect week

Here’s how to use the information from some of the other person-centered thinking tools to plan out Steven’s perfect week.

Step 1

Start with relationships

Start by thinking about who is important to Steven and how often he sees them.
 
You should be able to find this information from his one-page profile and relationship map.


Step 2

Add the what and where

Now think about what Steven wants to do and the places that are important to him.
 
The one-page profile and community map will show you the activities that are important to Steven and give you an idea about how often he wants to do them and where these activities would usually take place.


Step 3

Add outcomes

As well as the day-to-day information, look at where Steven wants to be in a year’s time. What can you do to incorporate this into his life and make his ambitions a reality?
 
You should be able to find this information from his what is working/not working and good days and bad days.


Step 4

Check with the person

Once you’ve completed the perfect week you need to check back your understanding with the individual and make sure that it works for them.
