About this tool

When you are supporting someone on a daily basis, you are learning about them all the time; finding out what works well for them and what doesn’t. However, all too often this knowledge stays with you as there is no system for passing it on to others. The learning log tool is a way of recording this vital information so that it can be shared with others and acted upon.
 
This is particularly important when someone is being supported by more than one person.


How does the Learning Log impact the other Person-Centered tools?

Any of the person-centered tools can be updated with information from a learning log (or any of the other tools). For example, something that worked well could be added to the information on “how best to support the person” in their one-page profile.
 
It’s essential that you do not think of the tools in isolation, but recognize that they complement each other and help you gather all of the information you need to support someone to live a life that matters to them. It’s also important to keep a person’s person-centered description (e.g. support plan) up to date.
 
Remember, person-centered planning is a continuous process of learning and adapting as a person’s wants and support requirements change.
