Using the decision-making agreement

When you are creating a decision making profile, you need to:

  • Clearly describe the decisions that the person currently makes so that the person is not over-supported or under-supported
  • Include details about how others need to support the person to be involved in the decisions in their life even if someone else is responsible to make the decision
  • Identify who and how the final decision is made and the reason why
  • Think together about what it would take for the person to make more decisions in their life and have more control over the decisions they make,
  • Add the date that the decision making agreement was completed and who contributed
  • Add any actions that need to be taken and by whom
  • Make sure that there is a review date and that someone is responsible for this
