Record information

Person-centered planning seeks to develop a better, shared understanding of the person and their situation. It’s not about recording what someone can’t do; it’s about getting to know someone as an individual, the gifts they have and that others appreciate, and finding out what makes them tick. When you record information in any of the person-centered tools, it should:

  • Be free from jargon and service speak
  • Be written in clear statements
  • Give you a real sense of who that person is
  • Provide a capacity view of the person (what they can contribute)
